Where can I purchase a good spare key fob ? How would I program it and get key cut?

A locksmith is another (often lower cost) option.
 
I would go to the dealership.

The overwhelming number of locksmiths who say they can do it but actually can't, is staggering. Just look through any car forum, not just this one. Tons and tons of failure stories, one or two success stories.

Considering the very likely inconvenience of unsuccessful attempts, it's worth it to pay a premium and get it done right the first time.

Mark my words - Keys & TPMS = Go OEM. Every. Single. Time.
 
I shall count myself as very lucky then.
 
Just don't fall for the "buy a used fob" idea -- they lock themselves to the car they're programmed for and it's nearly impossible to undo. There are locksmith tools that can do it - you can watch youtube videos where they do so but it's an unnecessary hurdle.

Dealership = expensive but guaranteed to work. I bought an aftermarket fob for my Sonata and plan to program it with my ebay dealer tool at some point. I've already bought the required PIN from a friendly Russian vendor. (It's a lookup by VIN that dealers can do for free but locksmiths and others have to pay ~ $25 from various sources)

How much is it at a dealership to buy a new programmed keyfob?

I bought one online for my daughters Optima and took it to Batteries+ to program. Was WAY cheaper than going dealer route. Worked perfectly too.
