Headrest damaged, how would this happen?

I noticed the pad came loose on the passenger side front seat headrest on my 17k mi 2014. Car was previously used by Hyundai Motors for promotional stuff and they only put a few thousand miles on it. The active headrest release function is not activated, nor has the car ever been hit from behind. Internally there is damage to some plastic parts, but externally it’s perfect. How the hell could this happen? I was able to remove the pad entirely from the back half with the posts

Are those plastic parts damaged or could one have been defective? I don't imagine they open many at the dealer so they man not even be familiar with the internals.
 
Those loose plastic pieces were inside the headrest, they’re pieces of the components that appear to attach the top area of the pad to the back section, which are severely fractured and broken. I really doubt there was anything defective, the damage seems to be inflicted.

Same thing happened to my 2014 equus passenger seat headrest. Initially quoted over $1,000 by dealer to replace but they're now only going to charge for the headrest (still about $600.) How did you get this fixed or resolved? Thanks

Taped it with black duct tape in the interim, until I found a used one on eBay for around $150-200.

Awesome. Followed your advice and saved $800. Ebay seller was oemsurpluspartz. Note for anyone repairing on own: To remove the headrest, lower the seat as low as possible then extend headrest with button half way. Then put ur hand through the hole and yank it up very quick. Then just pull it up. Insertion is easy, I put mine in and it clicked in.

This is a manufacture defective issue as it has happened to me twice now. First to my driver side headrest and now a few months later for no reason, the passenger side headrest. I have a little over 25K on the 2014 Equus, no accidents or damage. I called corporate and the dealer and to date, over a two months, no one has called me back as promised.
