Nope, no vents closed off. What is happening is that the evaporator is icing over. It can happen with high humidity and lower cooling loads. You can reduce the chances of it happening by selecting recycle. Once it happens, you can clear it in a few minutes by turning the compressor off and leaving the fan on.
You may be slightly low on refrigerant, which makes this more likely to happen.
I don't think so. Although I'm sure that's possible, you wouldn't be getting cold air out of one side and warm out of the other.
In the second case above I simply switched off the climate control and switched it back on and everything was fine.
